The most serious risk with fat burners is not that they fail to work. It is that some of them work through a prescription drug that was withdrawn from the world market for killing people, added illegally and left off the label.
That is not a scare story. It is what laboratory testing of these products repeatedly finds.
Sibutramine was a prescription weight loss drug. It suppressed appetite by acting on serotonin and noradrenaline in the brain, and it worked reasonably well.
In October 2010 it was withdrawn from the United States market following FDA recommendations, and from most other markets around the same period. The reason was the SCOUT trial, which found increased cardiovascular risk. Case reports have linked it to sudden cardiac death.
It substantially raises blood pressure and pulse rate in some people. For anyone with coronary artery disease, heart failure, arrhythmia or a history of stroke, that is a serious danger.

The commercial logic is straightforward and ugly.
A genuinely herbal fat burner does very little. Customers notice nothing, do not repurchase, and leave bad reviews.

It works because it contains a drug, sold as though it does not.

This is the part people underestimate.

An undeclared ingredient is not declared. It is absent from the label by definition. No amount of careful reading detects it.
Detection requires laboratory analysis. Methods including thin layer chromatography with image analysis have been developed specifically to find sibutramine in herbal slimming formulations, because visual and label inspection cannot.
Packaging quality tells you nothing either. Products with professional packaging, batch numbers and official-looking certifications have tested positive.
Sibutramine is the most common but not the only one.

Diuretics produce rapid apparent weight loss by removing water. That is not fat loss, and it carries dehydration and electrolyte risks.
Thyroid hormones raise metabolic rate and are genuinely dangerous without medical supervision, affecting heart rhythm and bone density.
Undeclared stimulants, including high-dose caffeine and various amphetamine-like compounds, produce raised heart rate, anxiety and sleep disruption.
Laxatives create the sensation of something happening while removing nothing but water and stool weight.

That figure covers medicines moving through ordinary channels. Supplements face considerably less scrutiny than medicines.

None reaches the threshold researchers use for clinical significance. The meta-analysis covering many of these concluded there is insufficient evidence to recommend any of them for weight loss.
So a genuinely clean fat burner does close to nothing, and one that produces a dramatic effect raises the question of what is actually in it.
Certain patterns should stop a purchase.
If a herbal product is producing a powerful pharmaceutical effect, the most likely explanation is that it contains a pharmaceutical.
If you have taken a fat burner and experienced chest pain, palpitations, severe headache or unusual breathlessness, stop it and speak with a specialist promptly. Tell them what you took and bring the packaging.
The problem here is structural rather than about picking a better brand. Unregulated products cannot be verified by the person buying them.
