Gallbladder disease is the most consistently demonstrated serious risk of GLP-1 medication, and it is more common in people taking these drugs for weight loss than for diabetes.

That distinction matters, because most people reading about GLP-1 side effects are considering them for weight.

What the evidence shows

A systematic review and meta-analysis by He and colleagues, published in JAMA Internal Medicine in May 2022, pooled 76 randomised controlled trials involving 103,371 patients.

The findings were clear.

Why weight loss raises the risk more

All three operate together at weight-management doses. That is why the weight loss figure is 2.29 and the diabetes figure is 1.27.

What this means practically

It does not mean you should avoid these drugs. A 37 percent relative increase on a low baseline is still a low absolute risk, and it must be weighed against obesity's own substantial risks, including gallstones.

What it means is that the risk is real, foreseeable and worth watching for.

The symptoms to recognise

Biliary colic is the classic presentation. Pain in the upper right abdomen or just below the breastbone, often severe, frequently starting after a fatty meal, sometimes radiating to the right shoulder or shoulder blade. It can last from thirty minutes to several hours.
