The number you are quoted is almost never the number you end up spending. That is true of medication, of clinic programmes, and of supplement plans.
None of what follows is necessarily dishonest. Most of it is simply not mentioned, because nobody selling you something is incentivised to add up the total on your behalf.
The consultation costs
Initial consultation. Private clinics in Karachi, Lahore and Islamabad charge roughly Rs 2,000 to Rs 5,000 to be assessed and prescribed.
Follow-up visits. Dose escalation on GLP-1 medication requires review. Each step up is potentially another appointment.
Specialist referrals. If a thyroid problem, PCOS or another underlying condition is suspected, that is another consultation and another set of investigations.
Some programmes include physician review in the price. Many do not. Ask explicitly, because it materially changes the comparison between two apparently similar quotes.
The monitoring costs
Responsible prescribing involves testing, and testing costs money.
Baseline blood work before starting. Periodic repeats during treatment. Thyroid function where relevant. HbA1c and glucose for anyone with diabetes risk. Liver and kidney function.
None of this is optional if the treatment is being supervised properly. All of it is invisible in an advertised price.
The delivery and storage costs
Specific to injectable GLP-1 medication, and specific to this market.
Cold chain delivery. Semaglutide, tirzepatide, liraglutide and dulaglutide all require refrigeration. Proper refrigerated shipping costs money. Sellers who skip it are delivering product that may have degraded invisibly, which is a total loss you cannot detect.
Home storage. Fridge space is trivial. Load-shedding is not. An extended outage during summer is a genuine risk to a month's supply.
Needles and sharps disposal. Minor per unit, but daily injectables mean roughly 30 needles a month rather than four.
The wastage costs
Rarely discussed and genuinely significant.
Once opened, most GLP-1 pens must be discarded after a set period regardless of how much drug remains. Saxenda pens go after 30 days. During dose escalation, when you are using less per day, this can mean discarding drug you paid for.
Dose changes create the same problem. Move from one strength to another and any remaining stock of the old strength may be unusable.
The restart cost
This is the largest hidden cost in the Pakistani market, and it is worth understanding precisely.
If your supply breaks because your seller cannot source the next shipment, several things happen.
Appetite returns within days to weeks. Weight regain follows, since GLP-1 medication manages weight while taken rather than permanently resetting it.
When supply resumes, you usually cannot pick up at your previous dose. Tolerance to gastrointestinal side effects fades during a gap, so re-escalation from a lower dose is normal.
That means paying a second time for weeks you have already bought.
Two or three interruptions across a year can mean funding eighteen months of medication to receive perhaps nine months of continuous effect. The advertised monthly price told you nothing about this.
The duration cost
The most consistently underestimated figure.
STEP 1 measured its results at 68 weeks, published in the New England Journal of Medicine. SURMOUNT-1 ran 72 weeks. SCALE ran 56.
People budget for a month, or perhaps three. The trials describe more than a year of continuous treatment.
The correct calculation is your expected mid-course monthly figure, plus every cost above, multiplied across the months you would realistically need.
The programme-specific costs
Clinic programmes may charge separately for meal plans, body composition scans, group sessions or maintenance phases after the initial course.
Meal replacement plans often price a starter pack attractively and rely on ongoing product purchase thereafter. The starter price is not the running cost.
Supplement stacks accumulate. A fat burner becomes a fat burner plus a fibre supplement plus a protein powder plus a multivitamin, each individually modest.
Gym memberships and trainers are frequently bundled into a plan or recommended alongside it.
The costs that are not money
Worth naming, because they affect whether you finish.
Time. Appointments, pharmacy visits, tracking.
Side effect burden. Nausea and disrupted digestion during escalation affect work and family life.
The cost of failing again. Repeated unsuccessful attempts carry a real psychological price, and they make the next attempt harder to begin.
The questions to ask before committing
Ask these of any programme, including this one.
- Is physician review included in the price or charged separately?
- Are follow-up consultations included?
- Is blood work included?
- Is delivery included, and is cold chain maintained where relevant?
- What happens to the price when my dose increases?
- What happens if supply is interrupted?
- What is the total for the full recommended duration, not the first month?
- Is there a maintenance phase, and what does it cost?
A programme that answers these clearly is easier to budget for than one quoting a single attractive monthly figure.
