Mounjaro is the most expensive way to lose weight in Pakistan, and the price you are quoted for your first month is not the price you will be paying by month six.
That is not a sales tactic by whoever is selling it. It is how the drug is dosed, and understanding it is essential before you commit.
Why there is no official price
Mounjaro is tirzepatide, made by Eli Lilly, approved for type 2 diabetes. The same molecule is sold as Zepbound for weight management.
Neither is registered in Pakistan. There is no approved local supply, no regulated price and no official distributor.
Everything in the country arrived through import channels, personal imports, or informal medical suppliers. Each carries different costs and different markups, which is why quoted figures vary so widely.
Imported GLP-1 injections in Pakistan generally run Rs 20,000 to Rs 50,000 per month. Tirzepatide sits toward the upper end, and often above it, for reasons covered below.
The escalation problem
This is what catches people out, and it is specific to tirzepatide.
Mounjaro is dosed at 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg weekly. You start at 2.5 mg for four weeks, then step up by 2.5 mg at intervals of at least four weeks.
Higher doses cost more. So your monthly cost climbs as you progress.
Someone quoting you a price is usually quoting the starting dose. Reaching the top dose takes around twenty weeks, and by then you are buying six times the milligrams you started with.
Budget for the dose you will end up on, not the one you begin with.
The efficacy data reinforces why this matters. In SURMOUNT-1, published in the New England Journal of Medicine, average weight reduction was 15.0 percent at 5 mg, 19.5 percent at 10 mg and 20.9 percent at 15 mg over 72 weeks. The best results came from the highest dose, which is also the most expensive.
Why tirzepatide is scarcer than semaglutide
Both are hard to get here. Tirzepatide is harder.
Tirzepatide launched more recently and demand exceeded manufacturing capacity almost immediately. It has been supply-constrained since launch rather than falling into shortage later.
Peptide manufacturing capacity cannot be expanded quickly. Building it takes years, and existing capacity was already committed.
When global supply is tight, unofficial markets feel it first and hardest. Legitimate distribution channels in regulated markets get priority. What reaches informal import routes is whatever is left, at whatever price scarcity supports.
This is why tirzepatide pricing in Pakistan is both higher and more volatile than semaglutide pricing.
What moves the price
The exchange rate. Every unit is bought in dollars or euros first. Rupee weakness feeds through within weeks.
Global supply. Tightening international supply raises unofficial market prices sharply.
Your dose. Covered above, and the largest single factor over a full course.
Chain length. Each intermediary between manufacturer and you adds margin.
Which brand. Mounjaro and Zepbound contain identical tirzepatide at identical doses, but they are priced differently in the markets they come from, and that difference carries through to import cost.
The costs beyond the pen
The injection price is what people compare. It is not the full figure.
Physician consultation. Private clinics in Karachi, Lahore and Islamabad charge roughly Rs 2,000 to Rs 5,000 for an initial consultation, with follow-ups during escalation.
Cold chain delivery. Tirzepatide requires refrigeration. Proper cold-chain shipping costs money, and sellers who skip it are delivering a product that may have degraded in transit.
Baseline testing. Responsible prescribing involves blood work before starting and periodically after.
Supply gap losses. The most damaging cost. If your supplier cannot source your next dose, your course breaks. Appetite returns and restarting usually means re-escalating from a lower dose, which means paying again for weeks you have already bought.
Realistically, the effective monthly cost lands 20 to 40 percent above the quoted injection price.
The duration nobody budgets for
SURMOUNT-1 measured its results at 72 weeks. That is roughly eighteen months of uninterrupted weekly injections.
The correct way to assess affordability is to take your expected mid-course monthly figure, add consultation and delivery, and multiply across the number of months you would realistically need.
Most households doing that arithmetic honestly find the total is beyond what they can sustain, particularly through exchange rate movement and supply interruptions.
A course abandoned at month four costs you the money spent and returns very little, because weight regain after stopping GLP-1 medication is common.
The counterfeit question
Where demand outstrips legal supply, counterfeits follow. Tirzepatide's persistent scarcity makes it a prime target.
You cannot authenticate a pen by appearance. Packaging, batch numbers and holograms are copied convincingly.
The practical checks are whether the seller can show a verifiable import trail from the manufacturer, whether refrigerated handling was maintained throughout transit, and whether payment is being demanded in advance through a personal account. If those answers are unclear, the product is unverified regardless of presentation.
What is actually available here
The honest position is that Mounjaro is a genuinely excellent drug that most people in Pakistan cannot reliably obtain, verify or afford across a full course.
That is not a criticism of the medicine. It is a description of the market.
