Saxenda often appears cheaper than the weekly injections when you compare pen prices. That comparison is misleading, and the reason is straightforward once you see it.
Saxenda is injected every single day. Weekly drugs are injected once. A pen containing a fixed amount of drug empties seven times faster.
Why per-pen pricing misleads
A Saxenda pen contains 18 mg of liraglutide in 3 ml.
At the full maintenance dose of 3.0 mg daily, that pen provides six days of treatment. You need roughly five pens a month.
Compare that with a weekly semaglutide pen, which typically covers four weekly doses, meaning one pen a month at maintenance.
So the relevant question is never what one pen costs. It is what a month costs, and on that basis Saxenda's apparent price advantage largely disappears.
Anyone quoting you a per-pen figure without explaining the consumption rate is either not thinking it through or hoping you will not.
The escalation period
Saxenda builds up over five weeks: 0.6 mg, then 1.2 mg, 1.8 mg, 2.4 mg, and 3.0 mg from week five onward.
The early weeks use less drug, so your first month costs less than your steady-state months. Budget for the maintenance figure, not the introductory one.
This is a shorter and simpler escalation than tirzepatide's twenty weeks, which is one genuine advantage. You reach full dose and a stable cost quickly.
What you get for the money
This is where the value calculation turns against Saxenda.
In the SCALE trial, published in the New England Journal of Medicine, liraglutide 3.0 mg produced an average 8.4 kg loss over 56 weeks against 2.8 kg on placebo.
That is a genuine result. But semaglutide averaged 14.9 percent body weight reduction in STEP 1, and tirzepatide 20.9 percent in SURMOUNT-1. In the STEP 8 trial, which compared weekly semaglutide against daily liraglutide directly, semaglutide produced significantly greater weight loss.
So with Saxenda you are paying a broadly comparable monthly amount for a weaker outcome and seven times the injections.
The costs beyond the pens
Physician consultation. Rs 2,000 to Rs 5,000 initially at private clinics in Karachi, Lahore or Islamabad, plus follow-ups.
Cold chain. Liraglutide requires refrigeration before first use. After opening, a pen is used within 30 days.
Needles. Often overlooked. Daily injection means around 30 needles a month rather than four.
Wastage. Once opened, a pen must be discarded after 30 days regardless of remaining content. At lower doses during escalation this creates genuine waste.
Supply gaps. With daily dosing, running out has an immediate effect. There is no long half-life smoothing over a missed week the way there is with semaglutide.
The adherence cost
There is a cost that does not appear on any invoice, and with Saxenda it is the significant one.
Real-world discontinuation rates for daily injectable medications are high. People manage the first month or two, then the daily routine becomes the reason they stop.
GLP-1 appetite suppression depends on consistent drug levels. Missing doses on a daily drug creates immediate gaps. Money spent on a course you abandon at month three returns very little, because appetite returns and weight regain after stopping is common.
When assessing Saxenda, the honest question is not whether you can afford five pens a month. It is whether you will still be injecting daily in month eight.
Availability in Pakistan
Saxenda is not registered in Pakistan. Whatever is available arrived through import or informal channels.
Availability has been inconsistent. Novo Nordisk shifted manufacturing focus toward semaglutide products as demand grew, which affected liraglutide supply in several markets.
Liraglutide is a somewhat lower-profile target for counterfeiters than semaglutide and tirzepatide, simply because demand is lower. That is a relative advantage, not a guarantee. The same verification questions apply: can the seller show an import trail, was refrigeration maintained, is payment being demanded in advance to a personal account.
Where Saxenda genuinely makes sense
There are narrow cases worth acknowledging.
Someone who has tolerated liraglutide before and responded to it has a reason to prefer it.
Daily dosing allows faster adjustment. If side effects are a problem, you can reduce or stop and see an effect within a day rather than waiting out a week-long half-life.
And availability sometimes decides it. A drug you can actually source beats a better drug you cannot.
The arithmetic that matters
Before committing, work out the total rather than the monthly figure.
Take your steady-state monthly pen count, add needles, add consultation and delivery, then multiply across the number of months you would realistically need. SCALE ran 56 weeks.
Then ask whether you will sustain 365 injections in a year.
