Zepbound and Mounjaro contain identical tirzepatide, made by the same company, at the same doses, with the same side effects. They are frequently priced differently.
Understanding why explains a great deal about how weight loss medication is priced globally, and it affects what you pay in Pakistan even though neither is registered here.
The two-brand system
Eli Lilly sells tirzepatide under two names.
Mounjaro is approved for type 2 diabetes. Zepbound is approved for chronic weight management in adults with a BMI of 30 or above, or 27 and above with a weight-related condition.
The molecule is the same. The dose range is the same, running 2.5 mg to 15 mg weekly. The escalation schedule is the same. The pen is the same kind of device.
Novo Nordisk does exactly this with semaglutide, selling it as Ozempic for diabetes and Wegovy for weight. Liraglutide follows the pattern as Victoza and Saxenda.
Why two prices for one molecule
Several forces produce the split, and none of them are about the chemistry.
Insurance treats the indications differently. This is the largest factor in markets with insurance systems. Diabetes medication is routinely covered. Weight management medication is frequently excluded as lifestyle treatment. A manufacturer prices into what payers will reimburse, and those are two different negotiations.
Different competitors. Mounjaro competes against other diabetes drugs. Zepbound competes against Wegovy. Different competitive sets support different prices.
Supply allocation. Separate brands let a manufacturer ring-fence stock. During shortages, diabetes patients are not left competing directly with weight loss demand for the same inventory.
Regulatory separation. Each brand carries its own approval, its own prescribing information and its own regulatory obligations.
Why this matters in Pakistan
Neither Zepbound nor Mounjaro is registered here. Everything arrives through import channels.
The consequence is that the price you pay here inherits the price structure of wherever the product was bought, plus the markup of every intermediary along the way.
If a supplier sources Mounjaro from a market where diabetes pricing is lower, that feeds through. If they source Zepbound from a market where weight management pricing is higher, that feeds through too.
This is part of why quoted figures for tirzepatide in Pakistan vary so widely. Buyers are not comparing like with like, even when the molecule is identical.
Imported GLP-1 injections here generally run Rs 20,000 to Rs 50,000 per month, with tirzepatide toward the upper end because of persistent global supply constraints.
The escalation cost
Zepbound is dosed from 2.5 mg up to 15 mg, escalating by 2.5 mg at intervals of at least four weeks. Reaching the maximum takes around twenty weeks.
Higher doses cost more, so your monthly outlay rises through that period.
The trial data explains why people push to the top dose. In SURMOUNT-1, published in the New England Journal of Medicine, average weight reduction was 15.0 percent at 5 mg, 19.5 percent at 10 mg and 20.9 percent at 15 mg over 72 weeks, against 3.1 percent on placebo.
The best results come from the most expensive dose. That is the arithmetic you are actually committing to.
The total, not the month
SURMOUNT-1 measured its results at 72 weeks, roughly eighteen months of uninterrupted weekly injections.
The correct calculation is your expected mid-course monthly figure, plus consultation at Rs 2,000 to Rs 5,000, plus cold-chain delivery, plus baseline and periodic blood work, multiplied across that duration.
Then subtract nothing for supply gaps, because those cost you more rather than less. A broken course means appetite returns, weight often follows, and restarting usually means re-escalating from a lower dose. You pay twice for the same weeks.
The brand arbitrage trap
One practical warning follows from all of this.
Because the two brands contain the same drug, sellers sometimes present one as the other, or describe a product vaguely as "tirzepatide" without specifying which brand or which market it came from.
That vagueness is not always innocent. It makes price comparison impossible and it obscures the supply chain, which is the one thing you most need to see.
If a seller cannot tell you which brand, from which market, with what import trail, you are not in a position to assess either price or authenticity.
What is obtainable here
The honest position on Zepbound in Pakistan is that it is the most effective approved weight loss medication in the world and it is not realistically available to most people here.
Not registered, not price-regulated, not reliably supplied, not verifiable, and requiring eighteen months of continuous purchase at import prices.
